Overview:
The Community Vital Signs Initiative, in collaboration with the San Bernardino County Department of Public Health, conducted the Community Themes and Strengths Assessment (CTSA) from November 2022 to January 2023. The CTSA aimed to gather insights from the community on health issues they face and to understand their experiences with health inequities.
2024 Community Health Assessment Report
Purpose of CTSA:
- Gather community input on prevalent health issues.
- Understand the impact of discrimination on health inequities.
- Use community feedback and data to set health priorities for the SBC Community Transformation Plan.

Community Health Survey and Data Findings
Findings:
- Preliminary findings from the survey were shared with the community in spring 2023.
- Analysis identified three Health Improvement Priorities presented at the 2023 Community Action Planning meeting in San Bernardino, CA.

Community Action Planning Meeting (August 1, 2023)
Overview:
A planning-for-action community engagement meeting was held on August 1, 2023, for community residents, partners, and stakeholders, to vet the goals and strategies that would be included in the SBC 2024 Community Health Transformation Plan.
The meeting aimed to ensure that community residents’ concerns and experiences were at the core of the planning process, by allowing them to voice their ideas and input.
During the meeting, key findings and health data were shared with SBC residents through a facilitated process that included community discussions and decision-making around identified health issues based on the 2023 CHA.
Community Engagement and Decision-Making:
- Residents discussed how the prioritized health issues impact various aspects of health and well-being.
- Strategies to address the three Health Improvement Priority areas were identified and prioritized by attendees, focusing on those that were reasonable, feasible, and reflective of their lived experiences.
Strategy Identification:
- The identified strategies obtained from the community’s input were compiled and refined to ensure they aligned with best-practices, were evidence-based and equitable.
- The strategies were aligned with the goals outlined in the 2024 CHA Report.
